Thanks for reaching out to us. Your balance looks like the Series 795. However, you'll notice that this uses a screw for installation while yours uses a 15-198 hook which means this must be a custom order. 

I'll email you a purchase link for (2) S795-32 balances with the 32C stamp and 15-198 hooks attached. The balances come attached with the 15-195 1-1/4" wide closed cam shoes but they won't work for you so you will also need to add the 15-195B open cam shoes to your cart separately. The video below will show you how to install them.

The white piece on the left in your first photo is called a brake which is a part of the broken T-lock shoe in your last photo. The other white piece is called a sash stop which usually goes at the top or the bottom of the window.
