A rep who now are the contact for our Simonton Luminess 400 series windows provided the following. Our current local window contractor/company stated we needed a new "balance shoe" for a double-hung window.
'The part number or balance sizes you need are 3.5 Omega and 5.5 Omega balances. They are for a Luminess 400 series window and the date manufactured is 6/2002.'

Thanks for writing in! The pivot shoe you have pictured here looks a lot like our 15-199 . I am confident this is the replacement you'll need but take a look at the dimensions below and compare them to your existing shoe to confirm.
This shoe is compatible with either the 1/2" thick Series 160 coil spring or the 3/8" thick Series 165 coil spring. To determine which series you need, you'll need to confirm the thickness of your coil spring. Once you've determined the series, you'll be able to to use the stamp from the coil(s) to identify the spring strength needed for your window.
If there are no stamps on the metal coil(s), you'll need to obtain the exact weight of your window and divide it by the total number of coils in the window.

Thanks for confirming. It does appear the pivot bar you need is the 26-378. If possible, you'll want to double check the measurements of your existing pivot bar. However, I am confident this is the replacement you'll need.
