Thanks for posting! The balance you have here is a Series 795 Channel Balance assembled with a 15-198 Balance Clip Attachment and the 15-195 1-1/4" T-Lock Shoe with a closed cam.

There is a chance this may need to be a custom assembly but that will depend on the shape of the installation hole at the top of the window frame. Should you find the hole to be square you can use the 26-200 Balance Screw Plug and 33-016 screw to install the S795-27 Channel Balance. There is a video tutorial below describing how to install them.

If the hole is circular, the replacements will need to be a custom assembly. Then, I'll need to know the exact number of balances you are looking to purchase to confirm pricing and availability.
