Need to buy hinges-Please identify

Dear Help Department,

I have an old commercial door that is dragging. I have narrowed it down to the top, left hinge. I would like to buy a replacement hing for the top and bottom of the door. Enclosed are pictures. I don't see any names or model numbers on the door or the actual hinges. I think that the building was built in the 1950s.

Please give your model number so that I might purchase the hinges. The door handle is on the right, and the hinges to the door are on the left.
