Thank you for reaching out to us. It looks like you have the Series 795 channel balance but the with 15-198 hook attached instead of the 19-043 screw tab. This configuration isn't a stock part of ours but we'd be happy to supply it via custom order. Let me how many balances you're looking to order and I can draft a price quote.

Please note, the Series 795 comes attached with the 15-195 closed-cam shoe by default. I'm not sure if this is what you have since yours is broken so you may want to check an undamaged version to confirm.
