Hello

With this combination of attachments, you could have either the S380-35 or S385-35 channel balance. You’ll need to measure as shown below. The Series 380 has a 1" bottom attachment, and the Series 385 has a 1-3/32" attachment. 

You'll also need 3440 spring strength options for your replacement balances. Thank you and let us know if you have any other questions or concerns.

My spring is 21" long. The bottom of the glide is 1 inch

The length of the spring is not used to identify replacement balances. The length of the metal channel, not including the end fittings, would be the determining factor. If you have other balances with 21" long metal channels, see the S380-21Otherwise, you would need the S380-35 (3440) balances based on your original photos.

In that case, the S380-35 (3440) channel balances would be the way to go.

So I need 4 of them. It is asking for an option. Mine would be 3440 correct.

Correct, 3440 would be the option needed for your balances based on your original stamp code. The option corresponds to the strength of the springs, which will support a sash between 23-30 lbs.

1 1/4 from underneath lip to center of hole. From front to back it's 1/2". The rest of them are ok

We aren't able to make out exact dimensions from photos but the measurements you provided are very close to ours. The 18-161 is also the only top sash guide in this style that we offer.
