Channel Balance

Question

I need a new tilt channel block and tackle. My broken part has 34C BSI RH with a T shoe. What part do I need?

See if the S795-34 matches up. Confirm if you need either the 15-195 or 15-194 T-Lock Shoe. Also confirm if your balance attaches to the top of the window track with a screw, or a hook like the 15-198 instead. If it's the latter, please show me the hole that the hook installs.
