I agree that your balance resembles the Series 795 channel balance. If the length of the metal channel is 29 inches long, the original stamp should translate to a 29E option that can support a 28-32 pound window sash. We recommend weighing the sash to confirm this range. 

It also appears that the T-lock shoe has an open cam style, which would be the 15-195B if your original measures 1-1/4" wide. This shoe would need to be purchased additionally and swapped out before installation.

Does your balance install with a screw or hook at the top? If your balance uses a hook, it will require a custom order to assemble the 15-198 hook at the top for you as the Series 795 comes standard with a screw in clip.
