Thanks for reaching out to us. I'm afraid we don't sell a match for this shoe. We have the 15-194 but this lacks a recessed stem like yours so it won't fit into the old grooved metal channel. With that said, our best option is to replace the balance entirely. See the Series 795 and the 15-194 shoe which is sold separately. This part just pops on and off as shown in the video below. There should be a stamp printed on the metal channel as well.
Please note that the S795 uses a screw to install at the top of the window track. If yours uses a hook, please let me know! We can still help vis custom order.

I'm afraid we don't. However, we can assemble your Series 795 balances with the 15-198 hook attached. I'll just need a few more details to get this moving for you. Can you tell me the stamp printed on the metal channel? How many balances are you looking to order?
