This looks like our 15-215 pivot shoe that's used in the Series 140 balance assembly. Review the specifications and let us know if you have any questions.

Do I replace just the shoe or the complete assembly with the coil

You only need to replace what's broken so you can re-use your coil springs if they're in working order. However, if they're not, see the S140-10 instead.
