Replacement Channel Balance - 30" Series 795?

Looking to order 2 sets of replacements for the channel balance(s) for my tilt out vinyl window.  The metal hook on one side has broken and I need that type, as opposed to the above-referenced one that has an eyelet.  

See pics which show the broken one, with the stamp 30D in the metal.   In addition to the channel balances, I'll need new shoes for each.  Current one measures 1-1/4" but perhaps I should also get the 1" ones just in case.

The hook has come off the one side a few times over the years and I've reattached myself, but will probably get my handyman to replace both of them once I receive the correct parts.

Here is another pic of the shoe.  From your site info, it appears to be a closed cam with a blue puck.  1-1/4" measurement is across the bottom at the widest point.
