I can't be sure with the hardware still installed but it seems like you may have a coil balance similar to the S140 shown below. Take a look and see how the specifications compare to yours.

We also offer the 15-215 shoe separately but I can't be sure if it would attach to your coil springs in the same way. I would personally opt to replace the entire assembly with the S140 but review both options and see what you think.
