Your balance shoe seems to be a match for the 17/32" thick 15-199 Pivot Shoe. To determine if this will work for your repairs, carefully look over the specifications and compare them.

The compatible pivot bar is the 26-378.

This shoe uses either the 1/2" wide Series 160 Coil Balance or the 3/8" wide Series 165 Coil Balance. You'll need to confirm the width of the existing coil to confirm the right series. 

Once you've confirmed the series, you'll need to look for a stamp/marking on the coils to select the correct weight option. If yours does not have a stamp, please see the tutorial tagged below this thread for instructions on how to select the correct coils.
