Triple arm crank outs

A home owner from Minnesota says:
I need replacement triple arm crank outs for two windows installed in the 1980's. Possibly Ply Gem windows. Need instructions for removing them and where to get parts that will fit. Thanks!

These operators are either our 39-253LB and 39-253RB, or our 39-438B and 39-439B. I recommend watching the video below to get an idea of the difference between these two pairs and determine which one you need.
