This appears to be a TRUTH EntryGard casement operator. From the picture, you can see it is stamped 45275. It has two arms, but, unlike any of the EntryGard operators I've found on the internet, one of the arms on this one (the arm which goes in the rail), is short and is attached near the hinge edge rather than near the edge which opens.
I am looking for both a LH and RH version.

Looking at your pictures, the operator shown only has one arm. The other arm below it is a part of the hinge assembly. Please start by reviewing the style and dimensions of the 39-035 and 39-036 operators to see how they compare to your original. Let us know what you think and if you have any additional questions.
