At first glance, this reminds me of our 39-220B and 39-221B operators. Review our dimensions and compare them to yours to see if this matches up to your original assembly.

Upon a closer look, there's another thought that comes to mind. It's tough to say since your operator is still installed but your operator style also resembles that of our 39-315 and 39-316, but it looks like your link arm has an offset. That would be more like our 39-220B and 39-221B, like I mentioned before.

Another idea is our 39-172 & 39-171. The offset goes in a different direction, but since the link arm is on bottom rather than on top I think it might make up for it. I can't be sure though. Look these over and let me know what you think.
