I think that the threads in the barrel are stripped. What parts do I need to fix this or does the whole thing need to be replaced?

Thanks for posting. Yes, when that 'worm gear' pops out with the crank handle, I am afraid that means the entire casement window operator has broken down and should be replaced. From what I can see, this operator might be either the 39-013 or 39-027E straight arm operators. Compare the dimensions below with what you have, and let me know if either is what you need. In either case, for a new handle, see the 39-114L sold separately.
