Thanks for posting. This looks like a T-Lock Shoe. You'll note that your old shoe has a groove in the stem, which interfaces with a similar groove in your channel balance. I am afraid this means you will need to replace not only your T-Lock Shoe, but your balance as well. 

See our Series 795. You'll need the length of the metal channel, as well as the stamp code on the metal channel to pick the right stamp option. If you're uncertain, show us a picture of your balance's stamp code on the metal channel, or get the exact weight of your window sash. 

You'll need to compare your T-Lock Shoe with our 15-195 1-1/4" shoe and our 15-194 1" shoe to see which is a better match. The 15-195 comes by default with our Series 795; but if you need the 15-194 instead, you can buy it separately and easily swap it out with what you need. The video below illustrates this process. 

Finally, it appears your balance attaches to the window track with a hook. Our Series 795 balances attach with a screw-on tab instead. However, because it appears your window track has a square hole, you can use our 26-200 screw plug to fit into the hole and then screw in the balance.
