Hello there. Your balance system is an older style that is no longer made.  It's a bit of a hybrid between two different pivot shoes commonly use in MI windows: the 15-199 and the 15-180. We don't have an exact match, or even a confirmed substitute, but I believe I can help narrow it down for you.

Between the two shoes I mentioned, the 15-199 is the best candidate. It's 17/32" thick and 1-1/4" wide, which I believe is the same as yours. You should also be able to use our 1/2" thick Series 160 coil spring. For the broken pivot bar, see our 26-378. Please review these parts and let me know what you think.
