Thank you for reaching out to us. This looks like a match for the Series 795, but it must be a custom order since it uses a hook like the 15-198. Can you confirm the width of the bottom shoe when measured as shown below? We'll also need to know the number of balances you're looking to replace.

Thank you! The Series 795 comes attached with the 15-195 1-1/4" wide closed cam shoe by default. If you can confirm the number of balances you'd like to order, we can get the ball rolling.
