Your balance resembles the Series 795 channel balance, but with the 15-198 hook at the top for installation. We can assemble this configuration for you via a custom order. 

Can you confirm if the T-lock shoe has an open (see through) or closed backing metal cam? How many balances are you in need of? For custom orders there is a minimum of (2) required. From there I can send over an order link with pricing for your review.  Thank you!
