Your operator resembles an old-style version of the 39-014 shown below. Take a look and confirm the specifications match. We also offer the 39-013 opposite-handed version if needed. See the 39-114L handles used with these operators.

As for the hinges, take a look at the 36-048 set. These should work as long as the 10" base that attaches to the window sill matches yours. Thanks for posting!
