Window operator quit working. Need to find replacement

Hello, I have a single arm window operator that doesn't work anymore. It is about 12 inch long. I need a replacement but could not find manufacturer information. Can you help to identify?

Bob, I want you to take a look at our 39-013 or our 39-014. Look over their dimensions and see if you think these will work as replacements.
