This looks like a match for our S795-27 channel balance but with the 15-198 hook attached on top rather than a screw clip. This configuration is not a stock option of ours but we can provide this balance via a custom order. We'll just need to confirm a few things before we proceed.

• The S795 comes with the 15-195 closed cam T-lock shoe. It looks like this may be what you have but you'll need to compare the dimensions to confirm everything matches up. 
• Can you tell me the quantity of balances that you'll need? Our minimum for custom orders is 2.
