Your balance resembles the Series 795 channel balance. Does your balance install into the window channel with a screw or a hook. This attachment should be at the end of the pulley cord.
Please also provide the number/letter stamped on the metal channel.
I cannot confirm from the picture, but is the metal cam in the T-lock shoe open (see through) or a closed backing style. Thank you!

By cam I assume you mean what window pin sets in? It is see through

You will need the S795-20 balance with the 15-198 hook installed at the top. We can provide this configuration via a custom order.
I have sent over a separate email with an order link for (2) balances with pricing for your review.
Please note that if your T-lock shoe has an open(see through) metal cam, you will need the 15-195B T-lock shoe. This shoe is 1-1/4" wide. Please measure to confirm before purchasing. This shoe will need to be added to your cart additionally from the product page before checking out and swapped out with the stock shoe before installation.
