Hello there! This looks like our Series 795 channel balance with a 15-198 hook attached on top rather than a screw clip. This is not a stock item of ours, so this will have to be a custom order. I'll just need more information before we proceed. Can you show us an undamaged version of this T-lock shoe? Can you show us a picture of the stamp on your metal channel? How many of these balances will you need? Our minimum for custom orders is 2. Thank you!

i've included the channel lock as well. i have another where the lendgth rod is 28 inches. not sure that matters since it looks like the lock only needs to be replaced. i would need 4 of each

Thanks for the extra details! This is a huge help. Like Emily said, what you have here is an S795 with a 15-198 hook, but you also have the 15-195B "open" cam pivot shoe.
