Hello. I believe the slides are made by Grass America and might be the Express model. These came with the kitchen cabinets that were installed in our home in 2013. The desk was purchased about 25 years ago from a Scandinavian furniture store in Massachusetts. Our house flooded from hurricane so hardware is corroded. Hello. This piece broke off of the back of the kitchen cabinet. It looks like it needs the two prongs. I've attached one of the brackets which still has the prongs attached.
