Please reply with the parts needed to fix the issue pictured.  The end of the handle tabs is broken off.

You can start by taking a look at the 79-127 and 79-128 commercial door pivots. This is used for Vistawall commercial doors. Compare the dimensions and mounting hole locations to see if they match up to yours. Also, take a look at the related video.
