This looks like the Series 795. Can you confirm how your balance installs? I can't quite make out the attachment in your third photo. Can you also show us a photo of the stamp on the metal channel? 

The Series 795 comes with the 15-195 1-1/4" wide T-lock shoe, so you'll have to use an undamaged shoe as a reference to confirm if it has a closed or open cam.

It sounds like your balance uses a hook like the 15-198 to install, rather than screwing in at the top of the window track. If that's the case, this must be a custom order. 

Can you confirm the stamp on the back of the metal channel? How many balances are you looking to replace? You said "three each" in your initial post, so I just wanted to clarify that.
