Here is what I think I need: Please verify from pics if you can...
Two Series 305 Spiral balances, 3/8" diameter, 16" with shoe attached: S305-16
Two spiral balance tops: I am not sure which one to use: I think the 19-012? If it is just to keep out dust it should work.
Two top sash guides: 18-034: Yours (looks like mine) says 1/2" wide. Mine is 7/16" wide. All else looks good. Will it fit?
Twist tool and instructions?

Many thanks for writing in, Warren. You are correct in choosing the S305-16 Spiral Balance. The 19-014 cap should work for you. Now, your old top sash guide is not available, but please compare your old guide's dimensions to our 18-034. You will need to modify this top sash guide by drilling a mounting screwhole in the guide and filing the top cap of the guide to accept the spiral balance. For a charging tool, use our 75-005. See the media guide below for tips on how to adjust the non-tilt spiral balance.
