Your window uses the Series 795 channel balance, but with a 15-198 hook at the top for install. This configuration will require a custom order to assemble for you. 

Can you confirm that the length of the metal channel excluding the nylon attachments measure 21 inches long? 

How many balances are you in need of? For custom orders there is a minimum of (2). From there I can send over an order link with pricing for your review.
