No, the 39-376 does not come with the part you're holding in your second photo. That's actually your operator's spline. That can't be replaced or repaired so you will need a whole new operator. We can help but we'd like to see a photo of your operator with the window open so that the arms that move the sash are fully visible.

So, like Emily said, you definitely need a new operator. The problem is that I can't quite make out what you have while it's still installed in the window, even with these new photos. Too many important details are obscured or out-of-frame. In order to identify these, we really need to see as much of it as possible; the less we see, the less likely we can ID a match.
For now, there are two pairs that come to mind that you may want to consider as a starting point. See our 39-253LB and 39-253RB (sold separately), as well as our 39-439B and 39-438B (also sold separately). The major difference between these two pairs is the style of the guide at the end of the arm. I'll share a video at the end of this post that shows you exactly what I mean.
Please note that these products are just an educated guess on my part. There is still a chance that you don't have either style. Just something to keep in mind. Furthermore, both styles are compatible with the 39-376 and 39-375 cover sets.
