s795 18in channel balance

Question

Hey my name is Mike, I need to locate series 795 channel balance 18in, it's the same as pictures home owner from Huntington Pa September 14 2011 can you help.

It's a NAMI sash window double hung slide and tilt.

We should be able to replace this balance, though it will have to be specially made. I'm thinking of using our S795-18 but with the top screw clip swapped out with a 15-198 hook. Before we can start, can you just tell me how many of these you need? Can you also tell me what the stamp reads?
