Replacement Top Sash guides

I posted earlier, but forgot to upload photos. I'm looking to replace these top sash guides. They're a left and right pair, with corresponding offset screw holes. For the measurements in the photos (in case they're not legible): height 1-7/16", top width 1-1/8", inner width 13/16", top depth 1". The ... (cont.)

Window hardware

Please find attached photos of my broken widow hardware. I've also included a picture from the opposite side showing the same piece of intact hardware. Can you give me the part numbers for this balance and top sash guide. I think they are Capitol single hung aluminum windows. Probably manufactured around 1995.
