Thanks for posting. The balance is most likely an S795-36, stamped 36C. The shoe is the 15-195B Open Cam T-Lock Shoe, sold separately. The process of swapping this shoe out with the one that comes attached by default to the Series 795 (our 15-195) is simple enough; see the video below. 

Before proceeding however, please show us how your balances attach to the top of the window jamb. If they use a screw clip, you should be good to go to purchase these items. If it uses a hook like the 15-198 however, this is not a stock design, but we can provide these balances for you via a custom order. We'd just need to know how many balances total you need. Great, that makes your life a whole lot easier then. I'm very confident in that case that our Series 795 will work as-is, just make sure to buy the 15-195B shoe separately so that you can swap them out.
