The screw on one side pulled loose and doesn't remain in place when window is cranked open or closed. On the window locks are the letters P.G.T. on top of the lock and Venice, Fl on the bottom of the lock. The stamp on the arm is no longer legible.

Thanks for reaching out to us. We're happy to ID the operator, but we'll need to see a photo of the arm that pushes the window sash. 

If you need the lock, we will need to see photos of how it latches to the window.

Thank you kindly! I'm unsure whether the hole in the vinyl frame or the mounting hole in the operator is stripped. You will have to inspect both holes carefully for any signs of wear or damage. You can also try installing the screw with the operator removed from the window to determine if the mounting hole is stripped. If the screw does not go in, then the mounting hole is likely stripped.

For a new operator, take a look at the 39-022 and 39-023, as well as the 39-476 and 39-477. These have a metal roller at the end of the operator arm, so you'll want to make sure yours does too. Review the specifications and see how they compare with yours.
