I'm sorry to say I cannot identify your balance shoe based on the photo of the coil balance support. I'll need to see a photo of the existing shoe to confirm its replacement match.

Thanks for the photo. This shoe is a match for the 15-199 17/32" thick pivot shoe. I'm confident it is a match but be sure to double check the specifications.
The compatible pivot bar is the 26-378.
For the coil springs, you'll need the 1/2" wide Series 160 Coil Balance. To select the correct coil weight option, you'll need to look for stamps/markings on the coil. If there are none, please see the tutorial (tagged below) for instructions.
