Trying to find the right part to fix my cabinets

I am looking for replacement brackets, see attaached photos.

IMG- peg 6582 picture depicts front of slide rail with a single prong footing. IMG-peg6583 picture depicts the rear of slide rail that fits in a support bracket which attaches to the rear wall of cabinet. Approximately 24'' long .

I've sent pictures of the original Blum hinge. The measurement on both the hinge and the plate is 1-3/4" center to center. I could not find hinges with that measurement.
