I think these may be Simonton windows, but I am not entirely sure.  This counter balance is broken.

Thanks for posting! Simonton windows use a few different coil balance systems. The best way to determine what coil balance system you need is to go by the pivot shoe, as the pivot shoe identity determines the coil springs. As I explained to your in our live chat, this shoe might be the 15-228, as it looks similar. But it's best to remove your old shoe and compare with the dimensions below to confirm it matches up. Let me know if anything differs.

The 15-228 works with This shoe works with either Series 160 Coil Balance springs, which are 1/2" thick, or Series 165 springs, which are 3/8" thick. You'll need to remove the old coil springs and measure their thickness to confirm which you need. See the video below that shows how to remove and replace this sort of coil balance assembly. 

All this hardware is modular, meaning it is sold separately. For the coil springs you have to choose spring weights, which designate the weight each individual spring can support. In the live chat you told me the window weighs 20 pounds. Be certain this is the exact weight. If so, we can deduce the springs weights you need. If your window uses the 1/2" thick Series 160 springs, you can use the (2) of the S160-10 10 pound springs, one for each side of the sash. If your window uses the 3/8" thick Series 165, go with the (4) of the S165-5 5 pound springs, a pair for each side of the sash. Let me know if you have any questions about this. 

Finally, your old coil housing can be reused if it is undamaged. Otherwise, see the 15-204 double assembly for the 3/8" springs, or the 15-150S for the 1/2" springs.

I have the hardware out.  The dimensions on the shoes are exactly as you describe.  The coil springs appear to be 3/8 in thick (see photo).  The pivot bars appear to be somewhat bent, so should I get these - https://www.swisco.com/3-Pivot-Bar/pd/Tilt-Window-Replacement-Sash-Pivot-Bars/26-378?

Assuming the coils are the right ones (see picture of side of coil) does this cart look like everything I will need?(see picture of cart)

Hi again. Yes, it's best to replace a pivot bar if it's bent. If the dimensions match what is shown below, then the 26-378 is indeed correct.

Furthermore, the 3/8" thick coils match our Series 165. So I think this is all just about ready to go. 

However, just one question: you had said the window weighed 20 pounds. I see you are puchasing (6) of the S165-5 5 pound springs, along with the 15-205 triple supports. Were all your original coil springs stamped '#5' and were there three on each side of the sash? If so, that would imply the window sash weighs 30 pounds. If 30 pounds is correct and (3) S165-5 5 pound springs on each side of the sash was what was in your window originally, and the balances worked fine prior them failing, then stick with it. I just wanted to clarify that sash weight before proceeding.
