How do you remove and then install a Lever type operator?

I have been trying to figure out how to replace a broken 39-069 window lever operator for years and now the time really has come! How the heck do you remove the crazy thing? The one pictured is a working window but identical to the broken handled one I want to replace.

How do lock a lever operator?

Crank out for awning

Looking for this crank out mechanism for this older style window . It works but it doesn't hold the window in completely. Like the window next to it does . It pulls in but you push the window and it wiggles out a half an inch. Please help thanks !
