Need plastic on the window to make it look like individual panes of glass

Balance specifications are 29" length x 1/2" width, stamp is 2820. Top fitting is ? length (broken) x 1/2" width x 1/2" depth. Bottom fitting is 1 1/8" length x 1/2" width x 1" depth.

I need full replacement parts for crank assembly for right and left window. There are no markings, numbers or brand or model anywheres on the window frame or glass. The arm is 9" end to end. The crank is 5" from mid screw to mid screw. see photos.

Trying to identify if the lower balance shoes are available. The top stops appear OK. We have not removed the sash channels the see if anything else is broken. If only the shoes are broken, are they replaceable without removing the sash channels?

Looking for The slide mechanism, or the whole unit if I have to. I bought this window at Home Depot in 2007. I sent a picture of the tag on the window, and also a picture of a card with the numbers on it.

the part number on the hinge is 348.258.00.32 the part that screws to the cabinet has 1 1/4 on it.
