Can you help me find the correct channel balance?

I have a broken window balance and I'm trying to find a replacement. I took pictures of all the pieces I know are relevant but let me know if you need anything else. Hello, I'm looking for a 33" Series 795 Channel Balance (33B) with the 15-198 Balance Clip already attached, as opposed to sold separately. I need the same thing for a 39" Channel Balance as well. Thank you for your help!

I just got new channel balances (S795-39 , 39D) and hooks (15-198). How do I attach the hooks to the balances?
