S745-26 26-1/4" Peachtree Balance

My windows are about 40 years old. The balances LOOK like a #745, except the plastic anchor fixture looks a bit different (see picture). On the back is marked "2521" (see picture). If this is, in fact, a 745, is it a -3, -4, or a -5? What is the difference?

Hi, I need to replace 6 or 8 of these balances. Need to know what is the part numbe to order. Also,any help on how to install would be great. I have tried to work on these and very difficult to get these back in place. Thanks
