We just need the lock for this sliding door

Hi I am having issues with door handle not locking and the top lock staying stuck.

Need replacement locking handle for Champion brand sliding door that was installed in 2011. The door is white vinyl but can't seem to find a model number. I can upload pics of the door and handle mounting area if that would help.

Sliding glass door is approx. 17 years old, guessing from sticker (see attached). There are 4 holes in door frame for handle (first three holes are about same size, last hole at bottom is smaller - see attached), but do not know if they're all from latest lock. Maintenance guy took off with broke ... (cont.)

We have the larger mortise plate... 11 3/4 top to bottom, 11" between top and bottom screws. Light gray color. We also need a replacement lock latch lever, also in the light grey color (we purchased a white one on line)

Please check the above photos and advise the part number and price of the replacement for the inside cracked handle. The center to center distance of the screws holding on this handle is 4". The width is 1" The total height is 9 3/4"

Looking for a full replacement for my old sliding glass door handle and lock. The handle has a 3 15/16 hole placement. The dual mortise lock measures 11 inches screw to screw.
