Aluminum Top Sash Guide, WT

Window isn't catching the balance in the right level, I think I need to replace one of the top guide. This window is using 30" Series 380 Channel Balance 2930. Can you tell me which one I need?

Please find attached photos of my broken widow hardware. I've also included a picture from the opposite side showing the same piece of intact hardware. Need help identifying this top sash guide. This is the straightest one I have, all others are bent. The top sash guide pictured attached to the window is bent.
