Do you have replacement parts for this kind of an opener?

I'm looking to replace both handle sets on a 4 panel sliding glass door unit where the two operable panels meet in the middle. The existing hardware is the hook latch style and the hole centers for mounting are 4-15/16".

We recently bought a house with 4 casement windows. Looks like the prior owner removed all but one crank handle and I'd like to purchase replacements. Can someone help me ID the right part?
