19-005 Channel Balance Top, Winged

I have a Nu Air non tilt window, single hung that has a damaged channel sash balance. The damaged part has the following ID Number: #29 5 fw cmc 10 Can you help me identify and order the correct replacement part?

I'm ordering S380-24. But, I need to replace a broken top sash guide, too. Which top sash guide is for S380-24? The weight of the sash is 13 lbs. Is 2330 the right option?

I'm looking at the stamp on my balance it does not say the full 4 numbers, it states "21 2" then after the "2" vertically it has 3 letter stamp "B.S.I.", then horizontally (same direction as "21 2" after the "B.S.I.") it's followed by a NG. We have a block and tackle that has a stamp 3030 HD B.S.I. It looks like your S390 but the stop shoe looks different. The top shoe on the bar that we have does not have wings. Yes. This balance comes fully assembled with the block and tackle pulley system installed as well as a 15-005 shoe and a 19-005 top attached.

The series 390 has 15-001 shoe and 19-002 top attached and the series 380 has 15-005 shoe and 19-005 shoe.
