I think this matches Product Code: 82-265. The only markings on the door that I could find is the gold label in the photos. I don't know the manufacturer name, but it was bought from Lowes in 2013. Just wanted to verify before ordering. Thank you.

This door was slammed shut in the locked position. That broke the top locking hook. All the dimensions fit your 82-265. This is a "Windoor" door which was made in Orlando FL.
