80-102 6' Sliding Door Track Cover, Small

This door was purchased ca. 2008 from Lowes. I can't find any helpful numbers or markings on the door. I need to replace both roller assemblies and the track. I have attached photos of the parts with measurements. Thank you for your assistance.

The side brackets that contain the two rollers measure 3.16 long x 1.06 high x .69 wide. There are two rollers that measure 1.25 diameter. The center distance between the two rollers measures 1.69.
