39-152 Andersen Classic Operator Cover

The photo shows my existing operator has three screws aligned with the geared pivot point plus two more screws on a smaller centerline toward the external side of the window. All replacement operators I see have a different mounting screw arrangement with two unaligned mounting screws with smaller ... (cont.)

I want to say that 39-301 is the hand crank we need but what is the plate that is needed to cover it? Thank you!

Windows installed probably 20 - 30 years ago. 2 windows per section x 3 sections with about 8" separation between each 2 window section along wall of upstairs room.
